Mar 2011 47120 0 Driving in the city, the car gave a "lurch" just before it was about to gear up. Then the engine light and AT lights came on. Stopped and turned off the car and the AT light went off. Wire had gotten loose going to the transmission. Wire tightened. Drove on the highway, at 110, the car lurched again and started to rev in the high 3000 - low 4000. Replaced transmission solenoid. When I picked it up, it "lurched" again, within 5 km's of the dealership but they were already closed.

Jan 2011 71340 20 Mud flaps on front tore and dangled. Made noise against wheels. Simply removed flaps, rather than repaired them. The brakes squeak very loudly under some circumstances. Mechanic said they didn't do it while they were driving but it wasn't a problem.

Sep 2011 96100 600 AC quit working - this shop doesn't fix AC units. Brake pads and rotors needed to be replaced, calipers needed to be repaired - they had come apart on two of the brakes.

Nov 2010 58000 260 Both rear shocks had blown - but did not have anything happen that would ordinarily blow them - heavy load + large bump/pothole. I believe the Mazda5 design stresses the suspension and hence shocks wear prematurely. 
Aug 2011 74000 275 A/C intermittent. My mechanic tested refrigerant, looked for leaks, air remained was cool. After another 48 hr I noticed a pattern:Temp gauge way up intermittently: a/c was cutting out because of engine overheating. New thermostat fixed it. 
Sep 2012 98000 882 A/T (Auto.Transmission) light came on; local garage found P0744 and P0841 codes; brought to dealer who said trans needed replacement. Have extend warranty;waited 2 wks for part. Only paid deductible; dealer found other issues-see other posts Also discovered broken #3 motor mount. Also discovered blown front struts. Paid separate deductible under extended warranty. Also discovered blown rear shocks when at dealer to replace transmission - but these were under warranty from my local shop; brought car there for replacement. Only paid labor. 
Mar 2013 108000 0 Car is burning synthetic oil at rate of 1 quart per 1,000 miles (1k miles.) Two different garages checked & found no leaks. They independently advised me to trade-in vehicle, as repairs for this are too extensive.

Jul 2008 0 Clockspring circuit failed within 2 hours of leaving dealership, causing horn to sound until it burned out. It was repaired under warranty, of course. From what I can tell, this is an extremely rare occurrence, but was very disappointing to say the least. 
Aug 2008 0 Gas cap broke - a known problem (bad lot of caps), and quickly replaced under warranty. Annoying and inconvenient, nevertheless. 
Jun 2009 9000 0 Faulty emissions valve - got stuck in open position. 
Aug 2010 26500 0 During routine maintenance, I had rattle coming from rear of car diagnosed as defective shock absorber. Part was ordered and will be installed in 2 weeks. All covered by warranty. Right rear shock. 
Dec 2010 33000 0 Left rear shock replaced.  Right front wheel bearings became noisy and had to be replaced. 
Jun 2012 65000 240 Rear shocks replaced again. Second time since I bought the car.

Oct 2010 44020 202 tire wear shows rear end toe in ( /- ). Noise in rear, esp over bumps. Shop found broken rear shock, and replaced bushings. Vent airflow direction controller tab on pssgr side/nr window fell off. Happened once before, but found part and re-installed. This time, replacing it altogether. Car burned 1 litre of oil since last service. Tipped up with 20W50 between oil changes. Told to monitor condition and see if continues.

Mar 2011 48980 600 Broken rear spring/shock and worn bushings. Other side done previously and front end will probably need it eventually.  Broken door handle on driver's side sliding door. Water gets into channel and can make door stick if it freezes. Handle broke when attempting to open door. Initially asked to pay for it, but was covered by warranty when owner asked. 
Aug 2011 53940 91 Car rear still making noise as a result of TSB manufacturing issue - weld bead interferes with spring movement, causing rubbing and damage. Weld bead needed to be smoothed to <2mm so it does not bind spring action. 
Jun 2012 58900 519 Broken shocks, again. Only got 25,000 miles on these ones. Company agreed to pay for the parts, but charged for the service. Broken engine mount. Engine mount redesigned. Covered under warranty (drivetrain 100,000km).
